ADULT HEALTH PROGRAM
Visiting Nurse Services offers home visits to chronically ill individuals who have no insurance, are underinsured or have health needs not covered under Medicare. Clients may pay a fee depending on their income. Registered nurses and a registered dietician make home visits to Polk County residents providing the following services:

  • medication set-up
  • assessing medication compliance
  • blood pressure monitoring
  • blood sugar monitoring
  • venipuncture
  • general assessments
  • communication with physicians and other health professionals
  • community resource referrals
  • health education
  • nutrition counseling

Interpreters are available upon request. Referrals to the program can be made by physicians, caregivers, family, friends, as well as self-referrals. If you or someone you know is in need of home health services call 515-558-9946.

AT HOME FIRST
Visiting Nurse Services offers geriatric care management to assist those in the community that want to stay in their home as long as possible. A nurse or social worker helps to connect older adults and their families to resources and services such as nurses, home health aides, companions, or housekeepers to help them remain independent and maintain a quality lifestyle. CASE MANAGEMENT SERVICES
Visiting Nurse Services offers assistance with medical and social needs to Polk County seniors who qualify for the Elderly Waiver Program. A nurse or social worker assists seniors in assessing their needs and finding resources to meet their needs in an effort to help them stay in their home as long as possible. The case manager helps seniors gain access to medical, social, educational, and other needed services. To receive case management services, call 515-557-9019.

SENIOR COMPANION PROGRAM
Visiting Nurse Services offers companionship and assistance with daily living needs to homebound adults through the Senior Companion Program. VNS recruits and trains adult seniors 60 and older who provide support to homebound adults to help them maintain a happy, healthy life at home. Whether you need assistance with picking up your medication; respite for a loved one while you run errands; or companionship, VNS has Senior Companions eager to assist you.
